The Services Offered by LifeGuard Home Health

Skilled Nursing Care encompasses but is not limited to the following duties: wound dressing changes, injections, blood draws, medication and pain management, and diagnosis-specific patient care. For example, a patient with hypertension will be educated about appropriate diet and exercise, as well as the signs and symptoms of high blood pressure. When patients are aware of how to prevent and manage their illnesses, they can better understand when to call their physician or when to take necessary medications.

Physical Therapy may involve establishing a home exercise program to strengthen limbs and joints to improve patient mobility; instruction in proper techniques for transferring in and out of the bathtub; training in the use of durable medical equipment (DME), such as a walker or cane; or effective pain management. The goal is to reduce patient effort and discomfort during activities, ambulation, and transfers.

Occupational Therapy may involve functional training such as appropriate techniques for lower and upper body dressing, correctly performing activities of daily living (ADLs) like bathing, dressing, and grooming, and inspecting the home for safety issues or risks.

Speech Therapy may involve teaching proper eating techniques to prevent dysphagia or exercises to overcome speech impediments.

The Medical Social Worker connects patients to the multitude of community resources that they are not normally aware of, including short and long-term counseling and financial planning. Other services range from dial-a-ride and in-home support services (IHHS) to applying for veterans' benefits or Medi-Cal/Medicare, and arranging for a higher level of care if needed.
The Certified Home Health Aide helps the patient with ADLs such as bathing, dressing, grooming, light household chores, meal preparation, assistance with transfers or ambulation, and medication reminders. The home health aide is a temporary source of assistance designed to transition the patient from a state of dependence to independently performing ADLs. If additional assistance is needed beyond standard home health services, a home care agency can provide a longer-term aide.

Interpretation Services provided through 3i Interpreting Inc.

Our Service Area extends throughout Alameda, Contra Costa, and San Joaquin counties.
